No special referral packet is required to begin. If the situation is urgent, include the deadline and use the form's context field so the coordinator can triage it correctly.

Submit the form at /partners/intake. Lanternmark opens the engagement from that record and follows up with the right people.
What happens after you refer
You receive confirmation that the referral arrived, with the submitted context and the next step clearly identified.
A Lanternmark coordinator reaches the family or the named point of contact within one business day to open the conversation and move the project forward.
You can request written milestone updates during the engagement, and the closeout records what was completed, what remains open, and where the handoff landed.
