- A discharge-day check-in call and a 72-hour review to catch what the discharge paperwork missed.
Good fit: A family has a confirmed (or imminent) hospital or rehab discharge and needs the home, equipment, follow-ups, and first 72 hours handled by one coordinator.
Not a fit: Pure in-home care staffing with no discharge coordination, ongoing weekly aide service, or post-recovery long-term care placements.
